These Terms & Conditions apply to all Medication Therapy Management (MTM) services provided by The Dose of Clarity. By booking or receiving MTM services, you acknowledge that you have read, understood, and agreed to the terms below.
1. Scope of MTM Services
Medication Therapy Management (MTM) is a professional pharmacist-led service focused on reviewing, optimizing, and improving the safe and effective use of medications.
MTM services:
- Include medication reviews, patient education, identification of medication-related problems, and development of a Medication Action Plan.
- Do not include medical diagnosis, prescribing, or emergency medical care.
- Do not replace consultation with a medical doctor or other qualified healthcare provider.
Any recommendations provided may be shared with your prescriber with your consent.
2. Patient Responsibilities
To ensure safe and effective service delivery, you agree to:
- Provide complete and accurate information about your medical history, medications (including over-the-counter and herbal products), and allergies.
- Inform the pharmacist of any changes to your medications or health status.
- Follow up with your prescriber as advised where clinical review or treatment changes are recommended.
The Dose of Clarity is not responsible for outcomes resulting from incomplete or inaccurate information provided by the patient.
3. Confidentiality & Data Protection
All personal and health information shared during MTM services is treated as confidential and handled in accordance with professional ethics and applicable data protection standards.
Information will only be shared with third parties (such as your doctor) with your explicit consent, unless required by law.
4. Fees & Payment
- MTM service fees will be clearly communicated before the session.
- Payment is required before or at the time of service, unless otherwise agreed.
- Fees may vary based on service complexity, duration, and follow-up requirements.
5. Cancellations & Rescheduling
- Appointments may be rescheduled or cancelled with at least 24 hours’ notice.
- Late cancellations or missed appointments may be non-refundable.
- The Dose of Clarity reserves the right to reschedule appointments in the event of unforeseen circumstances.
6. Follow-Up & Ongoing Care
MTM services may include follow-up support where indicated. The scope and duration of follow-up will be communicated in advance.
Additional MTM sessions or reviews beyond the agreed service may attract additional fees.
7. Limitations of Liability
MTM recommendations are based on the information provided, current clinical guidelines, and professional judgment.
Final decisions regarding treatment remain the responsibility of the patient and their prescriber. The Dose of Clarity shall not be held liable for outcomes resulting from decisions made by third-party healthcare providers.
8. Changes to Terms
The Dose of Clarity reserves the right to update these Terms & Conditions at any time. Updated versions will be made available on the website and will apply to future services.
